ENR3255 - Advance Care Planning, Letʻs Talk Story
Course Description
Participants will learn the importance of conversations about what matters most and the documents to ensure they receive the care they want. The course is designed for all individuals over the age of 18 and will include an opportunity to have their completed Advanced Health Care Directive notarized at not additional cost. Additional information included will be about resources to support individuals facing a serious illness and those at the end-of-life. The course will be 3 1-hour sessions.Learner Outcomes
By the end of the course, participants will be able to:
1. Understand the importance of meaningful conversations about health care decisions.
2. Learn about valuable resources to support individuals with serious illness and at the end-of-life.
3. Learn why everyone over the age of 18 should complete an Advance Health Care Directive.

Withdrawal and Refund Guidelines:
Please email the program coordinator and the registration office at kccocet@hawaii.edu to request for a refund. Please note that in order to receive a full refund, the request must be made at least 10 business days prior to the course start date. For a 50% refund, the request must be made within five to nine business days from the start date of the course. If the request is made less than 5 business days, no refund can be issued unless an exception is made by the program coordinator. Failure to attend class or notify your instructor of your intent to withdraw does not constitute an official withdrawal from the course. Agencies submitting purchase orders for student registration fees will be billed regardless of attendance, unless notification of withdrawal is made by the specific refund deadline.
